To help us stay on schedule, please have all the required registration steps completed prior to arrival.
- Insurance carrier information including policyholders name and date of birth
- Extraction Order or Office Referral
- HMO Referral
- X-rays
- Medical History including medications
- Medical release forms from; Cardiologist, Primary Care, Ect.
If you are unable to obtain any of the required registration materials, please contact our office so that we can make alternate arrangements, prior to your visit.
